#1030 — Track initiators of application submissions

Description

As we are tracking status changes from lead to in-review as Application Submitted by {initiator name} in the UI, we’d need to track who was the initiator in the logs/webhooks. This should have the initiator info as name, not just id. Note, the initiator can be the merchant, or a user, or admin (master). These should be separable, and name is only needed for users (partner/rep clicking the submit button)
